What should a babysitter emergency information sheet include?
At minimum: your phone numbers, a neighbor's number, your pediatrician, fire, police, the Poison Help line (1-800-222-1222 in the US), your home phone and your home address. That is the American Academy of Pediatrics' list. Add each child's allergies, medicines, date of birth and approximate weight.
The AAP's babysitting reminders on HealthyChildren.org (last updated August 2022) say that list "should always be left with a babysitter", and also suggest showing the sitter fire escape routes and leaving a note with each child's date of birth and approximate weight. Ready.gov's planning flyer for older adults makes the same point for any caregiver: a list of medicines, dosage instructions and allergies, plus "need-to-know information for first responders".
The printable below puts all of that on one page. Fill it in, check the top three are right, and stick it on the fridge.
Who should pick something else
- Your sitter is a nurse-level carer for complex medical needs. Use the care plan from your child's medical team. A one-page sheet is not a care plan.
- You want the sitter to get live updates or location. Family Ready doesn't track anyone and never calls or messages anyone automatically.
- You only ever need paper. The sheet on this page is free and works on its own. You don't need the app.
- You're on Android. Family Ready is iPhone only.
How the sitter sheet and the app fit together
- 1
Make an Emergency Card
Who to call first, allergies, current medications and important health information, in one clear card for each child. It never needs a subscription.
- 2
Open it in Emergency Mode
One tap shows a high-contrast, large-text screen with no ads, no pop-ups and no purchase prompts. It works without internet, as soon as you are past the lock screen.
- 3
Leave paper too
Print a dated Emergency Snapshot PDF for free. The app's own listing says to keep a printed copy, because no app can help if a phone is lost or flat.
That last step is a design decision we hold to: the free version always prints, because the sitter's phone is not your phone.
Babysitter emergency sheet questions
Is anything I type on this page saved or sent?
No. The sheet is built in your browser. Nothing is uploaded, and it is gone when you close the tab, so print it before you leave the page.
Should I write my home address on the sheet?
Yes. The AAP lists the home address as something to always leave with a sitter, because a caller has to give it to emergency services and a sitter may not know it by heart.
Where should the sheet go?
Somewhere the sitter will see it without asking: on the fridge or next to the door. Walk them through it once before you leave.

Emergency: call 911 (US) first. Poison Help: 1-800-222-1222. In a fire, get everyone out, then call from outside.

What the Emergency Card looks like

An honest limit: Family Ready organizes information you enter. It is not an emergency-response or medical service and does not contact emergency services. In an emergency, the sitter calls your local emergency number.
Paper on the fridge. A card in your pocket.
Print tonight's sheet, then keep the same details in a free Emergency Card that opens without internet.
